Neck and shoulder pain treatment in Lahore

gardan ka dard

Neck pain is what Dr. Mehwish names first when asked what she wants this clinic known for. It rarely stays in the neck: the pain spreads across the top of the shoulder, into the shoulder blade, and often down the arm — which is why so many people are treated for a shoulder problem they do not have. The headaches that come with it are treated as migraines for years when the neck is what is causing them.

Does this sound like you

What neck & shoulder pain feels like.

  • Stiffness turning your head far enough to reverse the car
  • Headaches that start at the base of the skull and come over one side of the head
  • Pain across the top of the shoulder, or a knot beside the shoulder blade that never releases
  • Pins and needles, numbness or heaviness running into the arm or hand
  • Shoulder ache that gets worse the longer you sit, and eases when you stand up
  • Waking up with it, or after a night on the wrong pillow
  • An ache that builds through the afternoon at a desk and eases by the weekend

What usually causes it

Mechanical neck pain
The commonest by far. Joints and muscles irritated by hours at a screen, a phone held low, and driving. It is painful and it is stubborn, but it is not damage.
Cervicogenic headache
A headache generated by the top joints of the neck. Typically one-sided, starting at the base of the skull, and worse on the days the neck is worse. Frequently mistaken for migraine.
Cervical radiculopathy
A nerve root in the neck irritated or compressed, sending pain, numbness or weakness down a line into the arm and hand rather than sitting in one spot.
Disc problems in the neck
A bulge or herniation in the cervical spine. Same idea as in the lower back, and it refers into the shoulder blade and arm rather than the leg.
Shoulder blade and trapezius overload
The muscles running from the neck to the shoulder blade held under tension all day by a screen, a shoulder bag or stress. This is the knot people spend years getting massaged without it ever staying away.
Whiplash
After a road accident, even a minor one. The pain and stiffness often arrive a day or two later, not at the time.
A shoulder problem, not a neck problem
Sometimes the shoulder really is the source — a frozen shoulder or a rotator cuff issue. The assessment separates the two, because they are treated completely differently and there is a separate page here for frozen shoulder.

You do not need to know which of these it is before you come. Finding that out is what the first assessment is for.

How we treat it

What actually happens, in order.

  1. Find out where it is actually coming from

Your first visit is a full 40–45 min with the doctor. We test neck movement, test the shoulder separately, check reflexes and strength in the arm for nerve signs, and work out whether the shoulder pain and the headaches are coming from the neck or from the shoulder itself. Those are the two questions most patients have never had answered.

  2. Hands-on treatment

Both doctors are certified manual therapists. The neck, the upper back and the shoulder blade are treated together, because a stiff upper back forces the neck and the shoulder to do work neither was meant to do.

  3. Settle the pain and the spasm

Therapeutic ultrasound, TENS, EMS, IFT, laser, dry cupping and heat are used to bring down the pain and the muscle guarding, so the neck tolerates movement and the exercises become possible.

  4. Strength, your desk, and your pillow

Deep neck and shoulder-blade strengthening with thera-bands, plus the specific fixes for your screen height, your phone and how you sleep. Neck pain that is not followed by this stage is neck pain that returns in a few months.

See a doctor first if any of these apply.

Physiotherapy is not the right first step for every problem. Call us and we will say so honestly, or go straight to a doctor if:

  • Dizziness, fainting, double vision, slurred speech or difficulty swallowing along with neck pain — this is an emergency, go to a hospital now
  • Weakness or clumsiness in the hands that is getting worse, or unsteadiness on your feet
  • Neck pain that started after a road accident or a fall onto the head
  • Fever with severe neck stiffness, unexplained weight loss, or night pain that wakes you

Who will treat you

Both doctors treat this.

Whichever time suits you, you see a Doctor of Physical Therapy who handles this condition every week — not an assistant.

Questions

Before you book.

Do I need an X-ray or MRI before I come?

No, and you do not need a referral either. The assessment tells us whether a scan is needed. Most neck pain does not require one, and getting a scan first usually just delays the treatment.

Could my headaches be coming from my neck?

Often, yes. A headache that starts at the base of the skull, sits on one side, and is worse on the days your neck is worse is very likely coming from the top joints of the neck. It responds to treatment of the neck, which is why it is worth having checked before you accept years of migraine medication.

Is my shoulder pain coming from my neck, or from my shoulder?

There is a simple test for it. If someone else lifts your arm for you and the shoulder still will not move, the problem is in the shoulder joint itself and it is likely a frozen shoulder, which is treated differently. If it moves freely when lifted but aches across the top and beside the shoulder blade, the neck is usually the source. The assessment settles it in the first session.

Will you crack my neck?

Manual therapy on a neck is graded joint mobilisation, not a forceful crack. Whatever is done is explained and agreed with you first, and nothing is done to a neck that has not been examined.

How many sessions will neck pain take?

Most patients need 5–7 sessions. You are reassessed as you go, so if you need fewer you stop sooner. Packages are available beyond 7 sessions if a longer course is genuinely needed.

What pillow should I use?

There is no single right pillow, which is why we will not sell you one. It depends on whether you sleep on your side or your back and on the width of your shoulders. The doctor will work it out with you in the session.

Gardan ka dard — can I be treated in Urdu or Punjabi?

Yes. Both doctors speak Urdu, English and Punjabi.

Can you treat my neck at home?

Yes, across DHA Phase 1, DHA Phase 2, DHA Phase 3 at Rs 3,000 per visit. Worth asking about if the drive itself is what sets the neck off.
